Subglandular implants weigh and after years, skin looses, and lengthens, breasts sag. What could be done then? Bigger implants or scary mastopexy? And then what? Ptosis again? Patients, especially young women should think about the disadvantages of subglandular breast augmentation!

Only the pectoral muscle can hold the heaviness of the implants.

Nowadays we do breast augmentation predominantly subpectoral using saline implants, which consist on an absolute harmless water-salt solution. These implants, especially when placed in a wide “pocket” below the pectoral muscle, are softer than silicone cohesive gel implants, and as patients say: they are moving quite natural - "as live", as they say. It is not necessary to wear a bra, or if used, busks should be removed because they can hurt the breast tissue and change the form of the new infra-mammary fold. Approaches for breast augmentation are several but in most of them patients complain that marks are visible, especially in supine position and arms up. The most harmless, the least noticeable (see photo) is the periareolar approach. This is the most atraumatic access and bloodless surgery, using the avascular approach, not hurting, without unnecessary loss of time, without significant pain. Patients are not limited socially and the next day they may appear in their social environment. Placed under the muscle, the implants do not drop as the muscle protects them. Due to the larger "pocket", the result is natural, the bust is soft to touch and take the natural body movement.
The operation lasts about 40 minutes. The shape of the breast is maintained permanently. The saline implants do not require replacement. Patients can safely travel by airplane, but should keep breasts from injury, unnatural pressure etc.

BREAST AUGMENTATION AND PTOSIS
In several cases of breasts ptosis, the closed Serdev suture lifting technique could be used, without scars, only by needle perforations, to lift the breasts to the level, suitable for subpectoral augmentation.
Another important application of the scarless Serdev suture lift is in cases of ptosis after subpectoral breast augmentation (after giving birth and lactation, aging process etc.). Both such lifting and augmentation solve most of the problems without scarring.

There are three possible areas for the insertion of an implant for augmentation. The lower fold of the breast is the most common site. It allows full excess and lives a hidden scar. The second site is through the areola. This incision gives the best esthetic results but it is also the only one that goes through the avascular plane of breast tissue therefore the risk of trauma nad infection is lower. The third one is through the armpits. Some surgeons prefer it because the scar is almost invisible (except for those women who like to wear open cloths, with no sleeves). The disadvantage of this incision is a slight asymmetry of the breast. The decision has to be made individually, with each woman and the cosmethic surgeon.
The augmentation implant can be inserted above or below the chest muscles. The below position is considered to be more preferable by most surgeons. There is less chance of feeling the implant, less chance of developing irregular folds or bumps on the surface of the breast and the implant is less likely to drop. This kind of cosmetic surgery technique is suitable for women with relatively little extra skin and less fallen breasts.
The augmentation implants come in two shapes: round and pear shape also called "anatomic implants" because their shape resembles the natural contour of the breast. They are more expensive and require larger excision during the breast augmentation procedure.

After the above the chest muscles augmentation the patient will be wearing a sports bra or a bandage. She will continue wearing them at all times, during day and night, for about a month. The patient will be able to take a shower 24 hours after the surgery. The most optimal results are usually seen 3 months after the surgery. Several things can complicate this plastic surgery procedure; those are rare complications, which you should be aware of. Bleeding around the area of the implant, infection, disturbances of sensation and touch around the nipples, silicon leak, rejection of an implant and some degree of irregularity on the surface of an implant."
